Surgical treatment of multiple cerebral hydatid disease

We retrospectively reviewed results of surgery and imaging findings of 36 patients with multiple cerebral hydatid disease (Ecchinococcus granulosus). Headache, vomiting and epileptic seizures were the predominant symptoms. Papilloedema and optic disk atrophy was revealed in 29 patients. In 7 patients hydatid cyst was also found in other organs. Common CT and MR1 findings of E. granulosus were well defined, smooth thin walled spherical homogenous cystic lesions with no contrast enhancement, calcification, and surrounding edema. One or more craniotomies with subsequent removal of cystic lesions were carried out in all the patients.
Multiple cerebral hydatid lesions entails extremely serious risk of fatal outcomes, but the advent of CT and MR1 significantly contributed to reduction of mortality from this pathology. Early detection of the lesions and well-timed surgery are of paramount importance to ensure favorable outcomes. Craniotomy should be large enough to evacuate cysts undamaged.References
