Abstract

Sciatica is most commonly caused by nerve root compression secondary to herniated disk. Rarely, it can be due to a lumbosacral vascular malformation. We present 43 cases with such a malformation, presenting as a chronic lumboradiculagia. The additional 3 malformations localized in cervical and thoracic region, presenting as a chronic myelopathy. Four malformation from 46 presented as acute hemorrhage. The patients were explored MRI, venospondylography and few cases with selective spinal angiography. The hemodynamic aspect of these cases are reported. Diagnosis of these rare malformations may be difficult. A multiplanar cross-sectional magnetic resonance scan can show characteristic abnormalities and may assist in recognition these malformations. Venospondylography confirms the diagnosis, allows to estimate hemodynamics of the malformation and help us to chose right surgical tactics. After surgery 80% patients shows moderate or prominent regression of their preoperative symptoms. The 20% of patients remains unchanged.
